Privacy Practices

Who We Are

Fitness Fuel Wellness is a telehealth practice operated by Carlo Pepia, ACNP-BC, a board-certified Acute Care Nurse Practitioner licensed in New York State. We are a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This Notice applies to all health information we create or receive about you in the course of your care.

How We May Use and Disclose Your Health Information

For Treatment

We use your Protected Health Information (PHI) — including lab results, health history, medications, and symptoms — to provide clinical services, design your protocol, and issue prescriptions where clinically indicated.

For Payment

Fitness Fuel Wellness is a direct-pay practice and does not bill insurance carriers. We may use your information to process payments, issue superbills upon request, or assist with HSA/FSA reimbursement documentation.

For Health Care Operations

We may use your information for internal quality improvement, compliance review, and practice management. This does not include selling your information or using it for marketing.

As Required by Law

We will disclose your health information when required by federal, state, or local law, including mandatory public health reporting, law enforcement requests, and court orders.

For Compounding Pharmacy Coordination

When prescriptions for compounded medications (including peptide therapies) are issued, we may share relevant clinical information with the licensed compounding pharmacy filling the prescription, as necessary for treatment.

Uses and Disclosures Requiring Your Authorization

We will not use or disclose your health information for the following without your signed written authorization:

• Marketing communications
• Sale of your health information
• Any use or disclosure not described in this Notice

You may revoke a written authorization at any time in writing. Revocation will not affect any disclosures already made in reliance on that authorization.

Your Rights Regarding Your Health Information

Right to Access

You have the right to inspect and obtain a copy of your health records. Requests must be submitted in writing. We will respond within 30 days. A reasonable copying fee may apply.

Right to Request Amendment

You may request correction of inaccurate or incomplete health information. We may deny the request in certain circumstances and will provide a written explanation.

Right to an Accounting of Disclosures

You may request a list of certain disclosures of your PHI made in the prior six years, excluding disclosures for treatment, payment, and operations.

Right to Request Restrictions

You may request limits on how we use or disclose your information. We are not required to agree, but if we do, we will honor the restriction.

Right to Confidential Communications

You may request we contact you by a specific method or at a specific location. We will accommodate reasonable requests.

Right to a Paper Copy of This Notice

You may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.

Our Duties

• Maintain the privacy and security of your protected health information
• Provide you with this Notice and follow its current terms
• Notify you in the event of a breach of your unsecured PHI

We reserve the right to update this Notice. Any changes will be posted on this website and made available upon request. Changes apply to all PHI we hold, including records created before the update.

How to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights:

We will not retaliate against you in any way for filing a complaint.
